We ask parents to fill out the Program Participant Enrollment Form at the beginning of each Scouting year. The purpose for gathering this information is to ensure that leaders have the appropriate knowledge to help your child participate in the activities of Scouts Canada’s programs. This information is used for the benefit of your child’s experience while involved in Scouting. Our leaders appreciate the private and sensitive information contained on these forms and they take appropriate precautions when storing and providing access to this information on a need to know basis.
The Membership Management System (MMS) is a database of all Scouting members and a web based application to register members.
We’d like to provide you with some background information about the various sections of the form. Please read the information below before filling out the form.
Participant Information: This section collects basic information on members such as name, address, etc. Here are some examples of how this information allows us to understand more about your child:
- Faith Affiliation: this information allows us to ensure appropriate programming in accordance with our principle of Duty to God and our programs spiritual emphasis. It also allows us to recognize dietary needs and to fulfill our desire to provide culturally sensitive programming and activities.
- There is also a check box for parents to indicate whether there are other medical, family circumstances, and cultural or faith requirements our leaders need to know for your child’s ideal experience in our program.
Medical/Emergency Information: This is important both to allow leaders to be aware of any possible needs in advance of a program activity (for example an allergy to insect bites), and to have the information available to assess and react should an emergency occur.
Emergency Procedures and Emergency Contact Information: This information ensures we can seek the best support for your child should an emergency occur. Our leaders will let you know as soon as possible in the event of an emergency that requires immediate medical attention for your child.
Privacy Policy Information and Photo Release Consent: Scouts Canada is committed to respecting the privacy of our members and their families. The check boxes allow you to choose not to have images of your child participating in Scouting activities used in Scouts Canada’s publications, ads and web site, to choose to not be informed of fundraising and other member benefits.
Parent/Guardian Involvement: The leaders in your child’s group are volunteers. The enjoyment your child receives from the program will increase with your help. We look forward to your support.
Consent to Participate: In this section, you are consenting for your child to become a member of Scouts Canada. Participation in Scouts Canada is voluntary and involves a certain degree of risk when participating in some of our activities. If you have any questions about the type of activities your child will participate in, please contact your child’s leader.

Photography Release form - Photography release permission is now included on the Youth and Adult Volunteer registration forms. Parents and volunteers will check the box if they do not wish any photos taken of them or their children to be used in Scouts Canada's publications. A copy of the signed registration form giving photo release permission will now be adequate proof of such permission for all Scouts Canada photo contests.
Photography Release Form for Outside Agencies. (For youth and adults). This form is to be used for special events which involve collaboration between Scouts Canada and an outside agency. It gives permission for us to share images with other organizations for use in their publications and web sites. (This must be used for any such events regardless of whether a parent has checked off the "photo release" box in the registration form). For further clarification contact National Communications.
